Family Health Centers, Inc. (FHC) is a non-profit community health center that aims to provide high quality primary and preventive healthcare services to all individuals, regardless of their ability to pay. Our mission is to offer the same level of care and attention to you and your family as we would want for ourselves and our own families. These services are accessible to everyone in the community. Family Health Centers is committed to the well-being of our community and provides a wide range of primary and preventive healthcare services at eight locations in Louisville Metro. This includes adult primary care, pediatrics, women's health (including prenatal care), dental care, integrated behavioral health services, laboratory tests, radiology, pharmacy services, and health education classes such as smoking cessation, aerobics, and diabetes care management. Additionally, we offer assistance with signing up for health insurance through kynect, Kentucky's health benefit exchange. Two of our sites, Family Health Centers - Phoenix and Family Health Centers - Americana, cater to special populations with unique and often complex health and social needs. Family Health Centers - Phoenix is a recipient of the federal Health Care for the Homeless grant and provides various clinical, psychiatric, and social services to the homeless population in the region. Family Health Centers - Americana houses a Refugee Health Program and serves a culturally diverse community of immigrants and refugees. Family Health Centers is accredited for ambulatory care and has been recognized as a certified Patient Centered Medical Home by The Joint Commission. We are proud recipients of the 2014 Torch Award for Large Nonprofits from the Better Business Bureau® serving Louisville, Southern Indiana, and Western KY.
